Please type appropriate information, then print form, and mail or fax to: Capital City Events, Society of Rheology 73rd Meeting, 9909 Thornwood Road, Kensington, MD 20895. Fax: (240) 363-0238.

Registration Fee: Short course registration includes a complete set of course notes. Cancellations received in writing by October 1, 2001 will be subject to a $30 administrative charge. No refunds will be granted after that date.

* Non-members who are registered to attend the 73rd Annual Meeting may register for the short course at the member rates.
⁺ Although every effort will be made to provide course materials for all participants at the start of the course, those registering late may not receive course materials on time.

Early registrations will be confirmed with a printed receipt by mail. Late registrations will receive a printed receipt in your conference packet. For registration verification, contact Maria Covell at (301)564-6200 or capcityevents@email.msn.com. Attendees will pay their own room, tax, and incidentals incurred by them at hotels. The federal tax identification number of The Society of Rheology is 13-620-3476.

The course will begin at 8:30 AM on Saturday, October 20, 2001.
